Frequently Asked Questions

To begin services a few things need to happen:

  • You must be referred to Choice Home Care (CHC) by your case manager for the specific service you would like to receive
  • CHC must conduct an intake to get general information so we can best serve you as a client
  • CHC must receive a correct and valid POS with your authorized service hours from the regional center
  • CHC will place a caregiver with your family, or if you have referred your own worker for us to hire for you, they will be able to begin working once we have processed their employment application and completed the necessary hiring steps.

Please contact Choice Home Care if you would like to refer someone you personally know and are comfortable with when it comes to providing respite or personal assistance for your family. Our office will provide you with an application and all other necessary paperwork.

If you are waiting for a POS to be sent to Choice Home Care by the Regional Center, CHC will contact you as soon as we receive it. If you would like to check in about a POS, you can call us at 818-894-4151 between 8:30 am and 5:00 pm, Monday through Friday. 

Yes. We insist that an orientation meeting be considered between the family and any respite worker. This allows everyone a chance to get familiar and comfortable together, before respite or personal assistance services are scheduled

Choice Home Care maintains usage logs based on timesheets received from respite workers. You should also keep a record of the dates and number of hours used each month.

You can call Choice Home Care to find out how many hours have been used and / or your current balance. Keep in mind that Respite Inc. cannot pay for time that exceeds the authorized hours.

Each caregiver submits a timesheet to record all dates and hours of service provided to a family. Hours are verified by parent / guardian (signature). Timesheets are submitted by email or fax. Timesheets must be turned in no later than 5:00 pm on the 1st and 16th of every month (even if it is a holiday). The delivery of timesheets within 30 days of providing the service is important in order to process them efficiently and make payment in a timely manner.
