Beginning 10/1/2025, we are switching to an updated Timekeeping Application that you must use to log your shifts. You should have received an email from Admin@choicehomecare.com with instructions on how to download and pre-register for the application. If you did not receive an email with these instructions, please contact admin@choicehomecare.com.

Caregiver Forms
  • EVV Time Adjustment Form
    • This form is no longer in use for caregivers in Los Angeles County. If you are having issues submitting time under the new FSL application, please use the contact information above to get in touch with our team.  If you provide services outside of LA County, you will receive an individual link via email. 
  • JFS Time Adjustment Form (For Senior Care client use only – This form will not be accepted if you do not provide services under Jewish Family Services)
  • Caregiver Time Sheet (Paper)
    • Caregivers can download Paper Time sheets here. Caregivers who are using EVV cannot submit Paper Time sheets and must use Choice EVV to Clock In/Out.
